Please make sure you fill out the travel meet waiver form and give it to your coach as soon as possible IF:

In case a parent or guardian is not attending the meet, you must appoint a chaperone.

In case a parent or guardian is not driving your swimmer during the meet, it is important to also name the driver you are permitting to drive your child during the meet:
